To request transcripts or education records, please contact your school’s registrar.
For patients of Tufts University School of Dental Medicine clinics, please contact the medical records department for copies of your records or to make a request to amend or restrict access to your record.

To submit a privacy-related question, request privacy training, or report a potential privacy breach or incident, please complete the following form (Tufts SSO required).
Privacy concerns may also be reported anonymously through EthicsPoint or by calling 866-384-4277.
Information security incidents must be reported immediately to the TTS Service Desk at 617-627-3376.
